	Enter 002 if you are taking the same seminar for a second time (we'll even honor this discount if you took the course from another company!) and you will receive a $100 discount. Be sure to arrive at the seminar with proof of prior attendance, and be prepared to fax this proof to 303-832-1997 prior to the session if requested to do so.
QUERI will provide a full refund of your fee, less a $50 administrative charge, if cancellations are made in writing (or by voice message) more than three weeks prior to the training. The registration is nonrefundable within three weeks of the training date. However, you may allocate your space to a colleague who has not yet registered for the training at any time for no additional fee. Please notify QUERI of such changes prior to the training date.
